I hereby grant Lake Area Industry Alliance (LAIA) and its member companies permission to use any photograph or video LAIA or its member companies have taken of me or in which we may be included with others, for LAIA's legitimate business purpose, without payment or other consideration to me, in perpetuity. I further grant LAIA or its member companies the right to use my name and my child’s name, voice and/or likeness. This permission includes the right to copy, print publish, reuse and/or modify any photograph or video, in whole or in part, individually or in connection with other material, in any and all current or future media, including the Internet and Intranet. I waive any right that I may have to review and/or approve the finished product. I hereby release and forever discharge LAIA and its member companies from any and all claims and demands, whether or not foreseeable, that may arise out of or in connection with the use of these photographs and videos, which release shall extend to any licensee, successor or assign of LAIA and its member companies and the photographer/videographer hired by LAIA or its member companies I am a legally competent adult and have the right to contract in my own name and my child’s name. I have read this authorization and release and fully understand its contents and that it is binding upon me, and my heirs, legal representatives and assigns.
